Reimbursement forms are due by December 1, 2017.

After filling out this form, please email all receipts or invoices to Meg Henke at mhenke@bridgeslibrarysystem.org. The subject line of your email should read, "2017 Library Improvement and Innovation Grant Receipts".

This should ideally be the library, not an individual.

If the reimbursement is to go to an individual instead of your library, please call Meg Henke at 262-896-8080 and provide your social security number so you can be entered as a vendor in our financial system.
